The following link is a request form for checking out balls during lunch. There is no guarantee you will receive the ball you request. Make sure you log into your NSD Google accounts in order to access the Google Form.
Everyday at 3:00 PM you will have access to sign up for the following day's lunch, and we will accept requests for balls until 8:00 AM the following day.
As of July 28, 2019, a new state law removes the personal and philosophical option to exempt children from the MMR vaccine required for school and child care attendance. Medical and religious exemptions are not affected by this new law.
